Sports & School Physicals

School Physicals: We now offer School Entry Health Exams in all of our clinics. We can complete the standardized forms given to you by your child’s school or daycare or we will provide the standard Florida State form. The School Entry evaluations are available on a walk-in, cash-only basis during regular clinic hours for a fee of $25 per child.

Please note: Appointments for School Entry evaluations are not available. In some cases, we must recommend that a child be evaluated by their PCP or a specialist to assure that your child can attend school or daycare safely. The responsibility of completing the clearance process lies with your child’s PCP or an appropriate specialist rather than with our clinic. School Entry evaluations cannot be done as part of an Urgent Care visit. If you feel your child may need an EKG, lab tests, x-rays, or prescriptions, please check him or her in as an Urgent Care patient for our evaluations. Immunizations are available through your child’s PCP or your local health department.
