Sports & School Physicals

School Physicals:

School and sports physicals available for ages 4 and up (children must be 4 years old on or before September 1).

Cash/credit only; insurance is not accepted for physicals.

Bring immunization records and all required school/sports forms to be completed.

Please note: We can complete the standardized forms given to you by your child’s school or we will provide the standard Florida State form.  In some cases, we must recommend that a child be evaluated by their PCP or a specialist to assure that your child can attend school or daycare safely. The responsibility of completing the clearance process lies with your child’s PCP or an appropriate specialist rather than with our clinic. School Entry evaluations cannot be done as part of an urgent care visit. If you feel your child may need an EKG, lab tests, x-rays, or prescriptions, please check him or her in as an urgent care patient for our evaluations. We do not offer immunizations; please visit your child’s PCP or your local health department.
