OFFICIAL CONTEST RULES: Television Giveaway
Participation: Participant must complete AHP’s Patient Satisfaction Survey and submit responses. In order to be eligible to win, respondent’s name and contact information (email address) must be submitted as part of the online survey form. Respondent will be entered to win upon the submission of each completed survey.
Winners: One winner will be selected at the end of each quarter (March, June, September and December) and will be notified via email.
Prizes: Winners will receive a 32” Flat Screen TV.
Eligibility: This Contest is open only to legal residents of the 50 United States and the District of Columbia (D.C.) and must be the legal parent or guardian of an After Hours Pediatrics patient, except immediate family (spouse, parents, siblings, children) of AHP employees. The Contest is void outside the 50 United States, D.C. and where otherwise prohibited.
