Every Child Makes a Difference
Program Information:
Every Child Makes A Difference® recognizes young people who have made exceptional contributions in their communities. These “Young Heroes with Big Hearts” may be in the age range of 5 – 21, and need not be patients of After Hours Pediatrics (hereby referenced as “AHP”). They are amazing kids who have reached out in some way to help others, and have made a difference.
QUALIFICATION:
- Children can be nominated for this award by parents, teachers, religious leaders, community organizations, physicians, etc.
- The nominee’s contact information and actions must be described in detail and must document how they have made a difference in their community, above and beyond what is normally expected.
- The nominee’s efforts should have a positive impact on others and have occurred in the last twelve months.
- The activity cannot be performed to solely benefit a family member.
- Winners are chosen based on effort, benefits and outcomes.
Winners/Awards:
Nominees may be in the age range of 5 – 21 years of age. Awards will be given in both the 5–11 and 12-21 year age brackets.
Regional winners will be selected twice a year in each age bracket, in each of the markets AHP serves: Tampa, Orlando and South Florida.
The winners will be presented with a unique engraved trophy, featured in AHP marketing and advertising, receive an amazing prize package and be honored with the naming of an actual star in the sky.
OFFICIAL RULES:
Eligibility: No purchase necessary. Nominees must be legal U.S. residents of the State of Florida. Nominees must be between the ages of 5 – 21. Each nominee must be nominated by someone age 21 years or older. Parents may nominate their own children. Employees of AHP, immediate family members (employees’ children) and members of the same household are ineligible for nomination. AHP associates may nominate non-related children and young adults.
Nominees may only be nominated once, and may not be a prior AHP Every Child Makes A Difference® regional winner. Multiple nominations for one nominee will not be accepted, even if submitted by different nominators. Awards are for individuals, so please nominate only one person per entry. Entries containing multiple names will not be accepted.
Nominations: Nomination forms may only be completed and submitted at www.everychildmakesadifference.com. No paper nomination forms will be accepted. Forms must be submitted and signed electronically by a nominator 21 years or older no later than November 30th, 2011. One nominator may nominate multiple students. All sections of the nomination form must be completed. Nominees, who become winners, and their parents, must sign a Publicity Release/Acceptance Form prior to receipt of any award. Nominations should describe how the nominee has contributed to the community in a meaningful way that has helped a non-family member. The activity must have been performed within the last 12 months. Financial need or academic performance will not be considered in the review and selection of the winners.
All nominations will become the property of AHP and will not be returned or acknowledged. Additional information about the nominee and/documentation may be requested.
Awards: Nomination forms will be accepted in two age groups: 5 – 11 and 12 – 21 years. AHP plans on recognizing one nominee from each age group, annually, from each site (regionally). Winners will receive an engraved trophy, as well as an amazing award package including the naming of an actual star in the sky in their honor. Additional prizes have not been determined and will not be consistent in name.
Notification: Winners of each age group will be notified regionally in early December, 2011. Non-recipients will not be notified.
Nomination Form and Process: Visit www.everychildmakesadifference.com, then click “Nominate”. You will need to read and agree to an online Consent and register as a nominator. During the nomination process, you will be able to leave the site by clicking on the save and log out button.
Correspondence to nominators will be via email only, check your email regularly. You must provide a valid email address that accepts bulk mail. To ensure delivery of messages, add info@afterhourspediatrics.com to your contacts, email address book or safe senders list.
Nominators may wish to gather the following information before beginning the nomination process:
- Information about the nominee: including full name, address, age, birth date, contact numbers, and current grade and school.
- Information about the nomination: including reason for nomination, time spent, explanation of people affected by good deed, outcome of good deed and general good deed description.
Nominators are solely responsible for submitting all information necessary for the nomination process. Nominations found to be incomplete as of the nomination deadline will not be processed. No faxed or emailed nomination materials will be accepted. No email attachments allowed.
All information submitted is considered confidential and is reviewed only by the AHP management team, responsible for managing the Every Child Makes A Difference® program. Information about the selected nominees will be provided to AHP for purposes of verification of eligibility, demographic research, publicity and recipient communication.
Carefully review your nomination before submission. All pages must be completed and entered into the format described in order to electronically submit the form. Standard capitalization must be used when entering data. Once the nomination is submitted no revisions can be made. The nomination must be submitted electronically by November 30, 2011. A confirmation email will be sent once you’ve electronically submitted your nomination.
Additional terms: Any related taxes on awards are the sole responsibility of the recipient. By participating, the nominators and nominees agree:
(1) To these Official Rules and all decisions of AHP which will be final in respect;
(2) To the use of their names, photos, likenesses, video, and/or images in any media, including without limitation on the internet for advertising, promotional, and publicity purposes in relation to this program without prior written consent or compensation where legal and subject to a signed a signed Public/Release Acceptance form.
(3) To release, discharge and hold harmless AHP affiliates, officers, directors, and employees, from any and all claims or damages arising out of, in any way, their participation in this program.
(4) AHP reserves the right to cancel this program at anytime, with or without notice and AHP reserves the right to interpret these rules and to make decisions concerning the awards and acceptance.
